We are hiring IMMEDIATELY for the following positions:
- RN Field Assessment Nurse (bilingual in Spanish desired)
- RN Care Manager ((bilingual in Spanish a Plus)
- Complex Case Manager Social Worker (fluent in Spanish for Queens and Brooklyn)
- Social Work Assistant-Field (BSW desired)
Education & Experience Needed:
Graduate of a 2- 4 year accredited nursing program, BSN preferred Active and unrestricted NYS RN License Minimum of 2 years Care Management experience with the geriatric population and/or minimum of 2-4 years clinical nursing experience in MLTC MAP and MLTC PACE preferred Medicare Managed Care experience (Complex Case Manager) New York State Driver’s License (Assessment Nurse, Social Worker) MSW, LMSW, preferred (Social Workers) Proficient in Microsoft Office (Word, Excel, Outlook)
